We accept and bill most Workers' Compensation insurance
carriers. If you have been injured, make sure that you have filed a report
of injury with your employer. If you have seen a physician one time you have
the right to change to another physician without having to file for a change
of physician form with the Industrial Commission of Arizona. Your employer
has the right to send you to a physician of their choice one time. After
that you have the right to choose a physician. If, however, your employer
is self-insured you must go to the physician which they direct you to. If
you have been treating (more than one treatment) with a physician and you
do not feel you are benefiting from his/her treatment you can fill out a
change of physician form with the Industrial Commission of Arizona to request
a change to a physician of your choice.
